The State of Vermont is offering you a new initiative to help you understand your health conditions and how to take better care of yourself. The Department of Vermont Health Access (You may know us as Medicaid, Dr. Dynasaur, VHAP, or PCPlus) has teamed up with APS Healthcare to offer the Vermont Chronic Care Initiative. This initiative is free to you.
The Chronic Care Initiative offers you a nurse to provide you with education on your overall health and ways you can feel better every day. Your nurse can help you to talk to your doctor about your treatment and can also talk with you and your doctor about how he/she can support your care plan. Our nurses work with a team of social workers as well as care coordinators and health educators to assist you with your health needs.
We look forward to talking with you soon. We will mail you information about your health condition, ways to talk with your doctor, and ways to take care of your health. You may reach APS Healthcare by calling toll free at 1-866-900-5004. If you are hearing impaired, please call using Relay 711.

Part of taking control of your health is knowing all there is to know about your condition. While you may have a chronic illness, you don't have to always be ill. There are daily changes you can make to feel better.
