Chronic Care Management Program
The State of Vermont is offering you a new program to help you understand your health conditions and how to take better care of yourself. The Office of Vermont Health Access (You may know us as Medicaid, Dr. Dynasaur, VHAP, or PCPlus) has teamed up with APS Healthcare to offer the Vermont Health Access Chronic Care Management Program. This program free to you.
The Chronic Care Management Program offers you a nurse to provide you with education on your overall health and ways you can feel better every day. Your nurse can help you to talk to your doctor about your treatment and can also talk with you and your doctor about how he/she can support your care plan. Our nurses work with a team of social workers as well as care coordinators and health educators to assist you with your health needs.
To get you started and help us better understand your specific needs, we will be conducting a health status survey. You are welcome to call us toll-free at 1-866-900-5004 and complete the survey over the phone. We have included a magnet with this phone number for your reference as well as information about your rights and responsibilities.
