When you agree to work with an APS Health Coach, you have certain rights and responsibilities.
Rights
I have a right to:
Get information about APS services and the Vermont Chronic Care Initiative.
Get the names and contact information of APS staff with whom I talk. I can also ask to talk with their supervisors.
Privacy of my health information. APS will only use my records for this initiative. APS can only release my information as permitted by State and Federal laws.
Be treated with respect.
Be treated as an individual.
Be involved in making decisions about my health. As allowed by law, a family member or guardian can represent me.
Speak with APS in my own language. If need be, APS will get me a translator for free.
Be told the rules for participating in the Vermont Chronic Care Initiative.
Choose not to be in the Chronic Care Initiative. If participating, I can quit at any time.
Be informed of future health benefits from the Vermont Chronic Care Initiative.
Upon request, get a written copy of my goals.
Complain about APS’ policies and to state my opinion without fear of punishment.
Responsibilities
I have the responsibility to:
Give APS and my doctor the information they need to provide me with services.
Follow my Vermont Chronic Care Initiative care plan.
Work with APS and my doctor to meet my health goals.
Understand my health problems as much as I can.
Notify my doctor of my involvement in the Vermont Chronic Care Initiative.
