The State of Vermont is offering you a new initiative to help you understand your health conditions and how to take better care of yourself. The Office of Vermont Health Access (You may know us as Medicaid, Dr. Dynasaur, VHAP, or PCPlus) has teamed up with APS Healthcare to offer the Vermont Chronic Care Initiative. This initiative is free to you.
The Chronic Care Initiative offers you a nurse to provide you with education on your overall health and ways you can feel better every day. Your nurse can help you to talk to your doctor about your treatment and can also talk with you and your doctor about how he/she can support your care plan. Our nurses work with a team of social workers as well as care coordinators and health educators to assist you with your health needs.
